This portal is designed to help you navigate through the onboarding process in chronological order and provide you with detailed completion instructions for important applications, forms, as well as anticipated deadlines for completion, to ensure your start date remains on track. Please take a moment to review the appropriate checklist based upon your credentials. This checklist will help you gather the information we will be requesting in the coming weeks.

New Provider Onboarding Requirements

Nevada Licensure Application

Controlled Substance Application

Physician Application

Print and mail the following items to the Board of Pharmacy:

  • Completed Controlled Substance (CS) Application – if your state license is pending, leave “Practitioner License Number” blank
  • $200 Registration Fee – must be personal check or credit card (credit cards are charged 5% processing fee)

APRN Application

If you need a Collaborating Physician per Nevada State Board of Nursing Board requirements, print and mail the following items to your Provider Recruitment Coordinator for submission. If you do not require a Collaborating Physician, please print and mail the following items directly to the Board of Pharmacy:

  • Completed Prescribe Application & Addendum – if your state license is pending, leave “Board of Nursing APRN Certificate #” blank
  • $200 Registration Fee – must be personal check or credit card (credit cards are charged 5% processing fee)

PA Application

All PA Prescribe applications require the original signature of the Supervising Physician. Print and mail the following items to your Provider Recruitment Coordinator for submission:

  • Completed Prescribe Application & Addendum – if your state license is pending, leave “Medical/Osteopathic Board PA#” blank
  • $200 Registration Fee – must be personal check or credit card (credit cards are charged 5% processing fee)

DEA Licensure

During your initial onboarding call with your Provider Recruitment Coordinator, you will discuss whether you will need to apply for a new DEA or transfer the address on your existing DEA to ensure you meet the necessary deadlines for credentialing.

New DEA Registration: once your application is processed, the Board of Pharmacy will issue you a pending registration number to apply for a new DEA. Please note: you will not receive your pending registration number until you have an active state license in Nevada.


Collaboration Agreement Forms

Note: The Collaboration Agreement Forms only apply to Advance Practice Providers (APPs).

Release of Information and ACO Consent Forms

Please print and sign both of these forms. Completed forms can be emailed to your recruitment coordinator directly.
The Release of Information will initiate your HR background check.

Credentialing Application

The Pre-Application Portal will assist Renown Professional Staff Services in providing you with the most appropriate set of instructions, application, and privileging criteria (if applicable).

You will receive two emails from the Renown Professional Staff Services team. The first contains the Medical Staff Initial Application Requirements, and the second contains your password information. Separate emails are sent for security purposes.

Use the password you receive to access the MSOW Portal and complete the online Nevada Standardized Credentialing Application. Please upload all supporting documents listed in the New Provider Onboarding Checklist with your completed Credentialing Application.

HR Onboarding

Approximately 30 days prior to your start date, you will receive an email from Renown’s Human Resources Department containing a link that will take you to your New Employee Paperwork. Complete all forms and hit “send to HR”.

Before your start date you will receive an email with instructions for your HR Onboarding Appointment. During this appointment you will sign your I-9.

Please bring with you two forms of government-issued identification.

Employment Physical

  • Contact Renown Occupational Health at 775-982-5000, and inform the scheduler you are a new employee.
  • Request an appointment for an “Employment Physical”
  • Appointment should be scheduled approximately two (2) weeks prior to, but no sooner than thirty (30) days before, your attendance of New Employee Orientation (NEO). Inability to complete appointment prior to Orientation will result in delayed start date.
  • Once your Employment Physical is scheduled, notify your recruitment coordinator of the date and time of the appointment
  • Employment physicals are held at Renown Occupational Health:

  • You can create an account through MyChart to access your records from your Employment Physical.
  • Required documents for appointment:
    • Immunization records (if possible)
      • Proof of MMR’s, varicella, and Hepatitis B
      • Titer’s (lab work) will be drawn on selected job classifications when no proof is presented.
    • Proof of a TB test within the last twelve (12) months.
      • If you have a history of a positive TB test, we require one (1) of the following:
        • MD statement of positive Hx
        • or actual documentation (card) of positive TB
        • or the most recent chest x-ray report (this must state CXR done for positive TB, otherwise one will be done)
      • Please bring eyeglasses on first visit for an eye exam.
      • Mask fitting or PAPR fitting is required for personnel with direct patient contact.
      • All employees will have a urine drug test on the first visit. (Please be well hydrated.)
      • New employee physicals will be conducted by an MD/DO. (Please wear loose clothing.)  Depending upon job classification and specialty unit, some employees will require more extensive work-ups and/or extra labs at the time of their first visit and will need to return for a second/follow-up visit.

  • How do I receive licensing reimbursement? After your start date, please submit all licensure receipts to your operations lead for reimbursement.
  • How do I get my Renown Provider Lab Coat?
    • All Renown providers are offered one white lab coat.
    • Site Leaders will assist providers in the completion of the Renown Provider Lab Coat Request Form.
    • The provider may take the completed form/certificate to the Renown Uniform Shop, pick out any white coat they choose, and the Renown Uniform Shop will facilitate the embroidery to follow Renown’s standards.
    • NOTE: Providers are not required to wear a white lab coat.
